The National Health Authority issued Health ID will make it easy for you to securely access and manage your health data digitally and setup a PHR (Personal Health Records).
You can create your Health ID using just your Mobile Number or Aadhaar Number, through the link which is Aadhaar seeded: https://ndhm.gov.in
Follow the steps shown in the PDF and create your Health ID card.
Presenting this Health ID to a participating healthcare provider will allow you to receive your lab reports, prescriptions and diagnosis digitally from verified doctors and health service providers seamlessly.
